Service Description

We are pleased to welcome you back for your follow-up visit, which may be conducted either in person or via telehealth, depending on your preference. During this visit, we will continue to focus on your health goals, review progress, address any new concerns, and update your individualized care plan. Our goal is to ensure your ongoing care is comprehensive, supportive, and aligned with your functional health journey. Time spent with provider is typically one hour. Please note that laboratory testing and related recommendations are not included as part of the visit. Price is prorated based on the time spent beyond the one-hour appointment. We do not accept checks.
